I'm looking for one clinician who genuinely wants to work with transgender, nonbinary, and otherwise gender-diverse clients. Not ‘willing to,’ not ‘affirming-adjacent’, someone who’s actively looking for this! I’m also open to hiring someone who wants to work with LGBTQ clients who are also ADHD and/or Autistic.

The work:

Individual therapy with adults and/or adolescents (your choice!), telehealth or hybrid in Boone, NC. You do sessions and notes- that's it! I handle billing, credentialing, insurance follow-up, intake, and the EHR.

The pay:

  • $93 per completed session, paid every two weeks.

  • For your first three months I pay after the claim pays out (usually 2-4 weeks), then I pay as notes are completed.

  • This is based on a percentage of the current rates, so as those increase, so will pay! The rate will increase the longer you’re with Fathom, and will increase if you’re seeing more than 10 clients a week. 

  • You get 100% of any no-show or late cancellation fees.

What you get:

  • Your caseload is yours. See however many clients a week feels good, no productivity review, nobody asking why you took two weeks off.

  • Free business coaching: marketing and self-promotion, PLLC setup, insurance panels, fee structures, building referral relationships, website SEO, running groups and workshops. I do this work professionally and you'd get it at no cost.

  • Free webinar series: Picking a health insurance plan, malpractice, credentialing, taxes for self-employed clinicians.

  • Want to pilot a group or a workshop? Pitch it. I'd love to help with the logistics and promotion.

Start/Keep your own practice:

It’s not required, but I want you to have one. I'll send sliding scale and private-pay inquiries to your own practice, rather than keeping them. Fathom handles insurance work only.

Accommodations are the rule, not the exception:

  • No recurring mandatory meetings. 

  • Written communication by default. 

  • Note deadlines extendable on request, and you don't have to explain why. 

  • Work whatever hours and days actually work for your body.

No weird vibes when you’re ready to leave:

No non-compete. You and your clients decide how the transition goes. If this stops being right for you, I want you to go do the thing that is.

Details:

  • Credentialing with BCBS and Aetna takes about 90 days, and we can't bill until it clears. If you're leaving somewhere else, plan to stay through the transition.

  • If you're building a caseload from scratch, keep another source of income while you do. Depending on your niche it can take a year to get to a consistent full-time caseload. The website brings in inquiries and I'll route the ones that fit you, but I'm not going to promise volume I can't guarantee. 

  • I am not a clinical supervisor, but I’m open to paying for your supervision if you’re seeing more than 10 clients a week. I also know many awesome supervisors in our community I can connect you with!

Requirements:

  • Licensure or Associate Status: LCMHC, LCMHCA, LMFT, LMFTA

  • Experience with LGBTQ+ populations: Demonstrable expertise in LGBTQ+-affirming care, including gender-affirming therapy, queer relationships, and trauma-informed approaches.

  • Tech-savvy: Comfortable using or learning how to use: EHR systems (we use Sessions Health), conducting Telehealth sessions, prompt email communication.

  • Self-motivated: Ability to manage a caseload independently while adhering to practice policies. Proactive in resolving scheduling issues, seeking consultation as needed, with a strong attention to detail.

  • Growth Mindset and Humility: Given the context of the populations we serve and the roles we hold, it is crucial that candidates are willing and interested in growing as clinicians and people. Investigating power structures and the systems we operate within are daily aspects of this role, and sometimes that includes learning where we’ve been making mistakes or overlooking our own privilege.
